What Is Project H.E.A.L.T.H.?
Project H.E.A.L.T.H. is a community-based health care program provided by Nixon Home Care, Inc., a non-profit organization, to enable the elderly and disabled adults to maintain an independent lifestyle in a well-supervised environment. This program provides clients with transportation, socialization, art and crafts activities, meals, snacks, and daily living assistance at Nixon Adult Day Center. It also provides nutritional and wellness workshops as part of our educational health program to assist each client with their health care.
Who Qualifies for the Program?
Individuals who are 65 or older, having just enough income to prevent them from receiving Medicaid, but not enough income to attend an adult day care program at private pay cost.
- Individuals who want or need some form of socialization and medical assistance during the day
- Individuals who cannot safely manage alone during the day while his/her caregiver works
- Individuals who are able to benefit physically, socially and emotionally from the day care service provided by Nixon Adult Day Center
