Our purpose is to provide the highest quality adult day and health services; meeting the needs of not only the clients, but also the caregiver and the community, by promoting the restorative and maintenance needs of our clients. This helps ensure that our clients can remain in their home and community environment, and maintain an optimum level of independence. Of great importance, the caregiver is given a respite and is assured that support is available so work and personal responsibilities can be met on a regular basis. Transitions will accept Private Pay and Medicaid clients.
